The lessor supplies the ownership document and the exact owner information for Form VSD-190. The lessor files Form RUT-25-LSE when it bought the vehicle from an out-of-state retailer not registered with the Illinois Department of Revenue, and the lessee files it if the leasing company does not. For a lessee who used the vehicle outside Illinois first, the Form RUT-25-LSE instructions subject the selling price to a reasonable allowance for depreciation for the period of prior out-of-state use. The lessee provides the lease, old registration, Illinois address, mileage, insurance, and any inspection proof.
